Working with Assisted Living Facilities
Doctor Jeannie Senior Care is a geriatric physician-led and owned medical practice that provides personalized, on-site healthcare for seniors living in assisted and independent living communities. We bring high-quality medical care directly to where you live, focusing on building relationships, simplifying medications, improving communication, and helping seniors stay healthier and out of the hospital. Our team works closely with families and facility staff to ensure coordinated, compassionate, and proactive care.
Your care begins with a full assessment by Dr. Jeannie Lessly May, a board-certified geriatric physician who will create a detailed care plan tailored specifically to you. Other companies charge up to $350 in addition to insurance for a geriatric evaluation, but DJSC has no additional charges over insurance. Once your geriatric care plan is in place, Dr. Jeannie's nurse practitioners and physician assistants will know exactly what to do. Follow up and acute visits will be made by your nurse practitioner or physician assistant. Your NP or PA will continue to collaborate with Dr. Jeannie.
Medicare expects a monthly visit for anyone in a facility setting so everyone is seen monthly. Additional visits are based on your individual needs. You will be seen weekly for an official visit when there is a medical reason to be seen such as uncontrolled symptoms or monitoring medication changes. Either you, your family, or the facility staff can ask for an acute visit sooner than monthly by calling 615-562-1411.
Unlike high-volume practices, our providers have smaller patient panels, allowing for more frequent visits when needed and ongoing Proactive Wellness Checks to catch issues early. The Proactive Wellness Checks are not official provider visits, but are part of our chronic care management program providing person-centered care.
Yes. We offer a transparent patient portal where you can view your full visit notes—not just summaries—so you and your family always know what is happening with your care. You can also send providers messages through the patient portal but these are not monitored all of the time so for quicker service please call 615-562-1411 - even if just to say you sent a portal message and please be sure it is addressed today. We will bend over backwards to help you out, but we need to know about it, and the best way to ensure that happens is to call.
